Ryan?s Story

Ryan is a healthy, active two-year-old, but his parents are concerned because he doesn?t seem to be doing the same things that his older sister did at this age. He?s not really talking, yet; although sometimes, he repeats, over and over, words that he hears others say. He doesn?t use words to communicate, though. It seems he just enjoys the sounds of them. Ryan spends a lot of time playing by himself. He has a few favorite toys, mostly cars, or anything with wheels on it! And sometimes, he spins himself around as fast as he does the wheels on his cars. Ryan?s parents are really concerned, as he?s started throwing a tantrum whenever his routine has the smallest change. More and more, his parents feel stressed, not knowing what might trigger Ryan?s next upset.

Often, it seems Ryan doesn?t notice or care if his family or anyone else is around. His parents just don?t know how to reach their little boy, who seems so rigid and far too set in his ways for his tender young age. After talking with their family doctor, Ryan?s parents call the Early Intervention office in their community and make an appointment to have Ryan evaluated.

When the time comes, Ryan is seen by several professionals who play with him, watch him, and ask his parents a lot of questions. When they?re all done, Ryan is diagnosed with autism, one of the five disorders listed under an umbrella category of ?Pervasive Developmental Disorders??a category that?s often referred to as simply the ?autism spectrum.?

As painful as this is for his parents to learn, the early intervention staff encourage them to learn more about the autism spectrum. By getting an early diagnosis and beginning treatment, Ryan has the best chance to grow and develop. Of course, there?s a long road ahead, but his parents take comfort in knowing that they aren?t alone and they?re getting Ryan the help he needs.

What are the Characteristics of Autism Spectrum Disorders?

Each of the disorders on the autism spectrum is a neurological disorder that affects a child?s ability to communicate, understand language, play, and relate to others. They share some or all of the following characteristics, which can vary from mild to severe:

  • Communication problems (for example, with the use or comprehension of language);
  • Difficulty relating to people, things, and events;
  • Playing with toys and objects in unusual ways;
  • Difficulty adjusting to changes in routine or to familiar surroundings; and
  • Repetititive body movements or behaviors. (1)

These characteristics are typically evident before the age of three.

Children with autism or one of the other disorders on the autism spectrum can differ considerably with respect to their abilities, intelligence, and behavior. Some children don?t talk at all. Others use language where phrases or conversations are repeated. Children with the most advanced language skills tend to talk about a limited range of topics and to have a hard time understanding abstract concepts. Repetitive play and limited social skills are also evident. Other common symptoms? of a disorder on the autism spectrum can include unusual and sometimes uncontrolled reactions to sensory information?for instance, to loud noises, bright lights, and certain textures of food or fabrics.

What are the Specific Disorders on the Autism Spectrum?

There are five disorders classified under the umbrella category officially known as Pervasive Developmental Disorders, or PDD. As shown below, these are:

  • autism;
  • Asperger syndrome;
  • Rett syndrome;
  • childhood disintegrative disorder; and
  • Pervasive Developmental Disorder Not Otherwise Specified (often referred to as PDDNOS). (2)

Although there are subtle differences and degrees of severity between these five conditions, the treatment and educational needs of a child with any of these disorders will be very similar. For that reason, the term ?autism spectrum disorders??or ASDs, as they are sometimes called? is used quite often now and is actually expected to become the official term to be used in the future (see the section below called ?A Look at ASD Diagnoses in the Future?). (3)

The five conditions are defined in the Diagnostic and Statistical Manual, Fourth Edition, Text Revision (DSM-IV-TR)? of the American Psychiatric Society (2000). This is also the manual used to diagnose autism and its associated disorders, as well as a wide variety of other disabilities.

At the moment, according to the 2000 edition of the DSM-IV, a diagnosis of autistic disorder (or ?classic? autism) is made when a child displays 6 or more of 12 symptoms across three major areas:

  • social interaction (such as the inability to establish or maintain relationships with peers appropriate to the level of the child?s development,
  • communication (such as the absence of language or delays in its development), and
  • behavior (such as repetitive preoccupation with one or more areas of interest in a way that is abnormal in its intensity or focus).

When children display similar behaviors but do not meet the specific criteria for autistic disorder, they may be diagnosed as having one of the other disorders on the spectrum?Aspergers, Rett?s, childhood disintegrative disorder, or PDDNOS. PDDNOS (Pervasive Developmental Disorder Not Otherwise Specified) is the least specific diagnosis and typically means that a child has displayed the least specific of autistic-like symptoms or behaviors and has not met the criteria for any of the other disorders.

Terminology used with autism spectrum disorders can be a bit confusing, especially the use of PDD and PDDNOS to refer to two different things that are similar and intertwined. Still, it?s important to remember that, regardless of the specific diagnosis, treatments will be similar.

How Common are ASDs?

According to the National Institute of Mental Health (NIMH) and the Centers for Disease Control and Prevention (CDC), some form of autism affects 2 ? 6 of every 1,000 children, with the most recent statistic being 1 in 110. (4) ASDs are four times more common in boys than in girls, although Rett Syndrome has only been diagnosed in girls. (5)

What Causes an ASD?

The causes of autism and the other disorders on the spectrum are not known. Researchers are currently studying such areas as neurological damage and chemical imbalances within the brain. These disorders are not due, however, to psychological factors or, as has been widely reported in the press, to childhood vaccines. (6)

A Look at ASD Diagnoses in the Future

In early 2010, the American Psychiatric Association released draft revisions to its Diagnostic and Statistical Manual of Mental Disorders (DSM-5) and invited comments from both professionals and the general public. The final and official fifth revision of the DSM is expected to be published in May 2013. (7)

When published, the DSM-5 is expected to affect how autism and associated disorders are diagnosed. Among the proposed revisions are:

Is There Help Available?

Yes, there?s a lot of help available, beginning with the free evaluation of the child. The nation?s special education law, the Individuals with Disabilities Education Act (IDEA), requires that all children suspected of having a disability be evaluated without cost to their parents to determine if they do have a disability and, because of the disability, need special services under IDEA. Those special services are:

Under IDEA, children with a disorder on the autism spectrum are usually found eligible for services under the category of ?autism.? In the fall of 2005, more than 160,000 school-aged children (3-21) received special education and related services in the public schools under the ?autism? category. (9)

IDEA specifically defines ?autism? as follows:

?..a developmental disability significantly affecting verbal and nonverbal communication and social interaction, generally evident before age three, that adversely affects a child?s educational performance.

A child who shows the characteristics of autism after age 3 could be diagnosed as having autism if the criteria above are satisfied.???? [34 CFR ?300.8(c)(1)]

Other characteristics often associated with autism are engaging in repetitive activities and stereotyped movements, resistance to environmental change or change in daily routines, and unusual responses to sensory experiences. The term autism does not apply if the child?s educational performance is adversely affected primarily because the child has an emotional disturbance, as defined in IDEA.

To have a child evaluated to see if he or she has a disability, including one on the autism spectrum, or to access early intervention services for a child up to his or her 3rd birthday, we recommend consulting NICHCY?s State Resource Sheet for your state. You?ll find a listing for early intervention under the first section, State Agencies. The agency listed there will be able to put you in contact with the early intervention program in your community.

To have a school-aged child evaluated for an ASD or to access special education services for a school-aged child, we recommend getting in touch with your local public school system. Calling the elementary school in your neighborhood is an excellent place to start.

What about School?

Early diagnosis and intervention are very important for children with an ASD. As we?ve mentioned, under IDEA children with an ASD may be eligible for early intervention services (birth to 3) and an educational program appropriate to their individual needs.

In addition to academic instruction, special education programs for students with ASDs? focus on improving communication, social, academic, behavioral, and daily living skills. Behavior and communication problems that interfere with learning often require the assistance of a professional who is particularly knowledgeable in the autism field to develop and help implement a plan which can be carried out at home and school.

The classroom environment should be structured so that the program is consistent and predictable. Students with an ASD learn better and are less confused when information is presented visually as well as verbally. Interaction with nondisabled peers is also important, for these students provide models of appropriate language, social, and behavioral skills. Consistency and continuity are? very important for children with an ASD, and parents should always be involved in the development of their child?s program, so that learning activities, experiences, and approaches will be most effective and can be carried over into the home and community.

With educational programs designed to meet a student?s individual needs and specialized adult support services in employment and living arrangements, many children and adults with a disability on the autism spectrum grow to live, work, and participate fully in their communities.

Tips for Parents

  • Learn about autism spectrum disorders?especially the specific disorder of your child. The more you know, the more you can help yourself and your child. Your state?s Parent Training and Information Center (PTI) can be very helpful. You?ll find your PTI listed on NICHCY?s State Resource Sheet (under ?Organizations Especially for Parents?). We?ve also listed? organizations at the end of this fact sheet that can help you become knowledgeable about your child?s disorder.
  • Be mindful to interact with and teach your child in ways that are most likely to get a positive response. Learn what is likely to trigger a melt-down for your child, so you can try to minimize them. Remember, the earliest years are the toughest, but it does get better!
  • Learn from professionals and other parents how to meet your child?s special needs, but remember your son or daughter is first and foremost a child; life does not need to become a neverending round of therapies.
  • If you weren?t born loving highly structured, consistent schedules and routines, ask for help from other parents and professionals on how to make it second nature for you. Behavior, communication, and social skills can all be areas of concern for a child with autism and experience tells us that maintaining a solid, loving, and structured approach in caring for your child, can help greatly.
  • Learn about assistive technology (AT) that can help your child. This may include a simple picture communication board to help your child express needs and desires, or may be as sophisticated as an augmentative? communication device.
  • Work with professionals in early intervention or in your child?s school to develop an IFSP or an IEP that reflects your child?s needs and abilities. Be sure to include related services, supplementary aids and services, AT, and a positive behavioral support? plan, if needed.
  • Be patient and stay optimistic. Your child, like every child, has a whole lifetime to learn and grow.

Tips for Teachers

  • Learn more about the autism spectrum. Check out the research on effective instructional interventions and behavior on NICHCY?s website. The organizations listed in this publication can also help.
  • Make sure directions are given step-by- step, verbally, visually, and by providing physical supports or prompts, as needed by the student. Students with autism spectrum disorders often have trouble interpreting facial expressions, body language, and tone of voice. Be as concrete and explicit as possible in your instructions and feedback to the student.
  • Find out what the student?s strengths and interests are and emphasize them. Tap into those avenues and create opportunities for success. Give positive feedback and lots of opportunities for practice.
  • Build opportunities for the student to have social and collaborative interactions throughout the regular school day. Provide? support, structure, and lots of feedback.
  • If behavior is a significant issue for the student, seek help from expert professionals (including parents) to understand the meanings of the behaviors and to develop a unified, positive approach to resolving them.
  • Have consistent routines and schedules. When you know a change in routine will occur (e.g., a field trip or assembly) prepare the student by telling him or her what is going to be different and what to expect or do.
  • Work together with the student?s parents and other school personnel to create and implement an educational plan tailored to meet the student?s needs. Regularly share information about how the student is doing at school and at home.

The Sessions Movie Review

Based on a true story, The Sessions stars John Hawkes as Mark O'Brien, a journalist whose lifelong polio affliction has left him paralyzed from the neck down. When Mark gets an assignment to write about the sexuality of the disabled, he panics a little, because he doesn't quite feel equipped: Mark is a virgin. In the name of research and, later, a newfound personal goal, Mark sets out to lose his virginity with the help of a sex surrogate, Cheryl Cohen Greene (Helen Hunt). While the premise may sound like an awkward mix of the raunchy and the emotionally manipulative, The Sessions avoids both. Yes, there is sex and there is poignancy, but at its core, The Sessions is a tender comedy with laugh-out-loud moments.

Though Mark O'Brien and Cheryl Cohen Greene are real people, The Sessions isn't a biopic acquainting you with Mark's entire life. Filmmaker Ben Lewin chose to focus on one specific time period and important experience in Mark's life (based on Mark's article "On Seeing a Sex Surrogate"). You don't need breadth to really feel like you've gotten to know Mark; all you need are the eloquent words of the dialogue and the excellent actors who deliver them. To find out what else I thought of The Sessions, just keep reading.

The backbone of The Sessions is Hawkes's performance. Indeed, after seeing him establish himself as sinister characters in other films, it's almost jarring to see Hawkes play someone so vulnerable and warm-hearted. But it takes no time to accept the actor as a severely disabled man, as Hawkes gives a 100 percent committed performance. It starts with the physical: Hawkes is lying down the entire time, never uses his appendages, and he uses a mouth stick for everyday tasks like dialing a phone as if he'd been doing it his whole life. It's impressive, but it's in the shadow of Hawkes's emotional performance. Though he incites a great deal of empathy, you never feel sorry for Mark. He's smart, he makes hilarious jokes, and he's believable as a man who can make women fall for him. Mark's disability recedes to the background, and the film simply becomes a story of a relationship between two people.

Hunt is also incredible as Cheryl, a sex surrogate who informs the audience ? just as she does Mark ? that she's not a prostitute. Her job is interesting and confusing, which makes it all the more enthralling to watch as Mark and Cheryl keep meeting and getting closer. Though Cheryl is very black-and-white in defining what her role is, their relationship enters a gray area as their interactions give way to real intimacy. It's more than sex, and both characters battle to figure out what that means. As a viewer, you're pulled into their same muddy thought process, and it feels bewildering and wonderful.

The overwhelming impression The Sessions gives is positivity. There's the sex positivity from Cheryl, of course, teaching Mark how healthy sexuality is, but there's also a generally uplifting feeling. The love and friendship present throughout ? from Mark's various friendships to his feelings for Cheryl ? fill you with unabashed joy.

Human rights groups decry Myanmar ethnic strife

SITTWE, Myanmar (AP) ? Western Myanmar appeared calm Saturday after almost a week of deadly ethnic strife, a government spokesman said as human rights groups called for action to end the violence that one said it has documented with satellite imagery.

Rakhine state spokesman Win Myaing said there were no immediate reports of fresh clashes between the Buddhist Rakhine and the Muslim Rohingya communities.

State television reported Friday night that 67 people had died, 95 been injured and 2,818 houses were burned down from Sunday through Thursday in seven of the state's townships. Win Myaing had earlier stated the death toll as 112, but later explained that had been a mistaken tally.

Amnesty International and Human Rights Watch issued separate statements calling for more government action to protect lives. Human Rights Watch released satellite photos that it said showed extensive destruction in a predominantly Rohingya area of one of the townships where violence was reported.

In June, ethnic violence in Rakhine killed at least 90 people and destroyed more than 3,000 homes. About 75,000 have been living in refugee camps ever since. Curfews have been in place in some areas since the earlier violence and were extended in scope this past week.

"These latest incidents between Muslim Rohingyas and Buddhists demonstrate how urgent it is that the authorities intervene to protect everyone, and break the cycle of discrimination and violence," Amnesty International's Asia-Pacific deputy director, Isabelle Arradon, said in a statement.

Human Rights Watch asserted in its statement that the Rohingya have suffered the brunt of the violence.

It quoted the group's deputy Asia director, Phil Robertson, as saying Myanmar's government urgently needs to provide security for the Rohingya. He added, "Unless the authorities also start addressing the root causes of the violence, it is only likely to get worse."

Human Rights Watch said the true death toll may be higher than that officially reported, based on witnesses' accounts and a history of government undercounting in cases that might reflect badly on it.

The Rohingya face grass-roots and official discrimination. Amnesty and Human Rights Watch both highlighted a 1984 law that effectively deprives most Rohingya of citizenship and denies them many basic civil rights.

Human Rights Watch released before-and-after satellite photos that it said showed the destruction of a Rohingya neighborhood in the coastal town of Kyaukpyu, where arson attacks reportedly took place Wednesday.

"The area of destruction measures 35 acres and includes 633 buildings and 178 houseboats and floating barges adjacent on the water, all of which were razed," said Human Rights Watch.

President Thein Sein's reformist government has described the violence as an obstacle to development. He took office as an elected president last year, and has instituted economic and political liberalization after almost half a century of repressive military rule.

"As the international community is closely watching Myanmar's democratic transition, such unrest could tarnish the image of the country," said a statement from the office of President Thein Sein published Friday in the state-run Myanma Ahlin newspaper.

The long-brewing conflict is rooted in a dispute over the Rohingyas' origins. Although many Rohingya have lived in Myanmar for generations, they are widely denigrated as intruders who came from neighboring Bangladesh to steal scarce land.

The U.N. estimates their population in Myanmar at 800,000, but the government does not recognize them as one of the country's 135 ethnic groups. Human rights groups say racism also plays a role: Many Rohingya, who speak a Bengali dialect and resemble Muslim Bangladeshis, have darker skin and are heavily discriminated against.

A statement issued Thursday by the office of U.N. Secretary General Ban Ki-moon called on Myanmar authorities "to take urgent and effective action to bring under control all cases of lawlessness."

"If this is not done, the fabric of social order could be irreparably damaged and the reform and opening up process being currently pursued by the government is likely to be jeopardized," it said.

The crisis has proven a major challenge to Thein Sein's government and to opposition leader and Nobel Peace laureate Aung San Suu Kyi, who has been criticized by some outsiders as failing to speak out strongly against what they see as repression of the Rohingya.

The U.N. warned Thursday that the crisis had sent a new wave of refugees to seek shelter in camps already overcrowded from the June violence.

Human Rights Watch deplored conditions in the camps, which it said housed many Rohingya who were denied access to adequate humanitarian aid and vulnerable to attack.

Apple posts rare earnings miss

Apple earnings are out, with Gene Munster, Piper Jaffray; David Pearl, Epoch Investment Partners; Daniel Morgan, Synovus Trust; Shaw Wu, Sterne Agee; and CNBC's Jon Fortt.

Fresh off its much-anticipated iPad mini reveal,?Apple?reported quarterly earnings that missed Wall Street's forecasts on Thursday and an outlook that fell shy of estimates.

Apple shares ended the regular trading day down 1.2 percent and were halted after-hours. After shares resumed trading, they fell more than 2 percent.?(Click here to get the latest quotes for Apple.)


Net income in the fiscal fourth quarter rose to $8.2 billion, or $8.67 per share, from $6.6 billion, or $7.05 per share, a year ago.

Revenue increased 27 percent to $35.97 billion from $28.27 billion a year ago.

Analysts had expected the company to report earnings excluding items of $8.75 a share on $35.8 billion in revenue, according to a consensus estimate from Thomson Reuters.

For the fiscal first quarter, Apple said it expects earnings of $11.75 a share on $52 billion in revenue. Analysts currently expect earnings of $15.43 per share and $55 billion in revenue.

"Relative to our numbers, they were kind of in line," said Shannon Cross, an analyst at Cross Research. "What people are going to focus on is that iPad was a bit weak and iPhone was solid during the quarter. The big question comes from the guidance.''

During the quarter, the company said it sold 26.9 million iPhones, 14 million iPads and 4.9 million Mac computers.

In the earnings release, the company announced that its board of directors has declared a cash dividend of $2.65 per share of common stock.

"The dividend is payable on November 15, 2012, to shareholders of record as of the close of business on November 12, 2012," the company's release said.

Shortly before the end of Apple's fourth quarter, the company launched the?iPhone 5, the latest version of its popular smartphone. Apple only had nine selling days of the new iPhone 5 in its fiscal fourth quarter, which means that all eyes are now on the current holiday quarter.

Since the iPhone 5's release in stores, the company's shares have slipped more than 13 percent from their $705 all-time high. Following the earnings release, the company's shares briefly fell below $600 for the first time since July 30.

The release comes two days after the company introduced the?iPad mini?and an update of its popular full-sized iPad. It said it has sold more than 100 million iPads since the devices debuted in April 2010.

How To Wear Your Wedding Dress On Your 20th Anniversary - Care2

My husband spent the last six months secretly planning a surprise wedding ceremony to renew our vows. I had no clue. Zip. I showed up in a t-shirt, shorts and a ponytail.? If I had even had the slightest suspicion, my hair and makeup would have looked more Cosmo crazy rather than country lazy.

Twenty years. That?s a lot of face time with one person.? We?ve traveled the world, had babies, changed jobs, changed homes, changed furniture. You know you?ve spent some serious time with someone when you?ve replaced all the appliances you received at your first wedding.

The night was romantic, sporadic, and casual. We laughed, we kissed, we danced, we ate? I cried. After twenty years of marriage what was the big question everyone wanted to know? how did I get into my wedding dress? Not one person asked us how we?ve managed to stay together for twenty years, which struck us as a bit curious.

I?ve had enough time to learn a thing or two about my body and my marriage. What I discovered was that the same lessons apply to both. The Skinny:

Acceptance

I accepted my body type before we were married. I?m never going to have broad shoulders, or trim hips or model long legs. It takes me forever to build muscle tone but then again I can eat a stack of Oreos without unsnapping my pants. ?No matter how hard I work out or how well I eat, my general frame is going to stay the same so striving to look like a gym goddess is an utter waste of my time. I?ve spent the last twenty years making ?Monica? the best I can possibly be.

The same can be said of my marriage. I?m never going make my husband into a jolly, devil-may-care guy, and he?s never going to make me competitive, so there?s no use trying. We?ve never spent a moment comparing our marriage to someone else?s or tried to be the poster child for marital bliss. Each relationship is a unique creature with its own energy and purpose so enjoy what it is, thrive in it and feed it.

Love what you?ve got. Strive to bring it to its fullest potential.

I find all the diet research incredibly frustrating. I?ve literally read an article advocating one thing to read another article pushing the exact opposite: take vitamins, don?t take vitamins, eat lean meat, go vegetarian, run a marathon, don?t run a block.? I believe, over the next decade, research is going to prove that ?diet? is not a one-size-fits-all formula. You?ve probably already noticed this in your own life. Your girlfriend lives off Ruffles while you gain a pound walking by the chip aisle. The expert that is going to know what?s best for you is the body you?re living in; forget Slim Fast, listen to your hips.

Over the last twenty years I?ve become an expert at listening to my body. When a New York State Fair blooming onion brought me to my knees at thirty, I stopped eating deep fat fried foods. ?I avoid cheesecake because rich food makes me nauseous. I never take Tums or Pepto-Bismol because they mask the messages my body is sending me.? I?ve learned to control my weight by what I eat and my health by the exercise I get.

One of the best tools for communication I?ve found is the scale. I get on the scale each morning so I can see how the food I ate the day before processed in my body. I?ve learned that carbohydrates, late dinners and alcohol bump up my weight while a solid breakfast, lean protein and green juice notch it down. And believe it or not, marathon training and long distance biking triggers my body to gain weight. You?ve got to stop looking at the scale as a torture mechanism so you can receive the daily personal messages your body is sending.

The same holds true in my marriage. Listening to one another is a big part of staying connected. True listening is done when you not only hear the words, but the needs behind the words.? When I say that I need him to help me with the yard work he hears ?I?d like your support.?? When he says he wants to decide what we are doing this weekend I hear, ?I want to be included in our family.?? We?ve learned that when one of us becomes defensive the communication is over.? He knows what matters to me and I respect the things he cares about. When we came to a shaky phase in our relationship we always, always turned to one another, even when the truth was painful to share.

Every successful relationship requires deep listening.

Apple not only released the iPad mini but also a brand new iPad 4 to sit on top of the tablet food chain. It's got the new Apple A6X processor, the new Qualcomm international LTE radio, and the new Lightning connector.

Does that chance the comparison much when it comes to the competition? Absolutely but not entirely. Google is rumored to be introducing a new Nexus 10 (made by Samsung, no less) as soon as next week. That's make for a much better comparison than the only current Nexus tablet, which is more in between an iPod touch and iPad mini in size and focus. Amazon has also just released the Kindle Fire 8 HD which now includes cellular data as an option -- though it won't be available in many countries regardless.

Microsoft's Surface RT is a wildcard here. Microsoft calls it no-compromises but that just means they punted the decision making to me, the consumer. I'm taking a wait and see attitude. It could be as great as the Xbox, or as doomed as the Zune. There's just no way to tell until it's been out for a while and the use case becomes clearer.

Likewise, while there have been rumors of a larger BlackBerry PlayBook for a while now, RIM is wisely concentrating on getting BB10 out the door, so I doubt we'll see it for a while. That leaves the 7-inch PlayBook at their only entrant in the tablet space, but you can find them for $149 and at that price, if you don't have a couple hundred dollars more, it's quite the value.

Still, Apple has sold 100,000,000 iPads to date, and that's an incredible amount for a product that launched in March of 2010. What they'll do next is a big question, but the iPad 4 is a better iPad, again, and anyone who wants to unseat it has their work cut out for them.
